I, We herby authorize you to release to USA FINANCIAL, LLC it's successors and/or assign, or their attorneys, all information requested pertaining to my credit, employment history, mortgage loans rating and history, or any information deemed necessary for the processing, approval and completion of my application.

A photographic or carbon copy of this authorization (being a photographic or carbon copy of the signature of the undersigned), may be deemed to be the equivalent of the original and may be used as a duplicate original.
